Terms of Business
Assessment fees
Fees for assessment are set on the website under the "Service" page. The fee of £250 includes an initial appointment of 90minutes and a follow up lasting 50mins.
The service requests part payment of the fee as a non-refundable deposit of £50 prior to the initial appointment. The remaining balance will be due at the end of the assessment process.

If you need to reschedule an assessment appointment please note that there will be an additional charge of £50 per appointment rescheduled.

Treatment fees
Where possible these are available on the website under the "Services" page.
​
Fees may vary with frequency of sessions, therapy or appointment type and the time of day.
​
Fees are reviewed annually but will not be changed during your first year of treatment.
​
Organisation Fees
These vary depending upon the nature of the work required and will be discussed with you directly and agreed in a contract.

Missed Sessions & Breaks
Upon agreement of regular session times, we secure the sessions for you and do not use this time for other purposes. Therefore fees are charged for the sessions allocated to you rather than those attended.

The service does not charge for any sessions that Dr Jeewa might cancel or when Dr Jeewa is not working over planned breaks.

Billing and payment
The service will issue a monthly electronic invoice at the start of each calendar month for your sessions in the previous month. Our preference is for fees to be settled by bank transfer. Where this is not possible we can accept cheque or cash.

Ending Treatment
Should you wish to end your treatment Dr Jeewa will discuss an end date with you. Usually this will allow time to reflect on the experience of ending therapy. Working towards an ending is a key part of therapy. In the event that this is not possible we require two weeks notice or payment in lieu.

Caution
Psychotherapy is considered an explorative process. It’s course can not be mapped out in advance. As such, the service is unable to guarantee results.
​
At times, therapy may give rise to negative feelings or acutely disturbing states of mind. Such experiences will be thought through with you. In the event that you require further support beyond the scope of the service, Dr Jeewa will discuss this you.
Privacy Notice
Dr Saleem Jeewa is the Information Commissioner’s Office (ICO) registered ‘data controller’ for the service. He is responsible for, and controls the service processing your personal information.

The type of personal information collected
Currently the service may collect and process the following information:
Personal details such as your name, address, e-mail address, phone number, date of birth, next-of-kin contacts, GP information and medical insurance information.
Details relating to your medical history including treatment.
Information provided by you or other health care professionals like assessment reports and referral letters.
The service may collect organisation details such as the name, address, contact details. It may also collect information provided by the company, it’s representatives and staff that informs and guides our approach in working with the organisation.

Why collect your personal information
We collect personal or organisation information on the foundation of our legitimate interests to provide healthcare and psychological services including administrative purposes.

How personal information might be used
Information may be used to carry out necessary duties in providing medical and psychological services to you.
​
Dr Jeewa may share information with other health care providers such as your GP, emergency services or other psychiatrists or psychotherapists who may be involved in your care. Dr Jeewa will only share that which is considered necessary to carry out our duties safely and effectively. Except in an emergency, he will seek your consent to share information before doing so.

Ensuring your consent to collect and share information
Under the General Data Protection Regulation (GDPR) you are able to remove your consent at any time. You can do this by contacting Dr Jeewa directly. Please note, Dr Jeewa will discuss this with you to consider how to continue your care, whilst ensuring safe practice. Where there is risk to your health and safety, Dr Jeewa may be unable to hold clinical responsibility without your consent to share information with others such as your GP and NHS Mental Health Services. In such circumstances Dr Jeewa will discuss this with you in order to find a way forward which is safe and supportive of your healthcare needs.

In case of an emergency or serious circumstance including risk to your health and safety or that of others, Dr Jeewa may share information with any of the following:

Emergency services
NHS Mental Health Services
Your Next of Kin
Safeguarding Teams
In such scenarios, Dr Jeewa will attempt to discuss this with you, prior to sharing information. This is in keeping with the expectations of all Psychiatrists holding a licence to practice in the United Kingdom.

The service may need to share information with:
Insurance providers.
The Court or other regulatory/public body if under a duty to disclose or share your personal data.
Our indemnity insurer and solicitors if a claim were to be brought against the Practice.
Data Processors who provide software and host our secure database which holds your data as well as our email service provider. Data processing agreements are in place to ensure your data can not be used or shared without our explicit instructions and will only be held for the period instructed by us.

Secure Storage of your personal information
Your information is securely stored online for 20years after your last contact with the service. The service does not hold any personal information on paper.

The service makes every reasonable effort to protect your personal information. However, we are all aware that information shared across the internet is not totally secure. Therefore, the service cannot guarantee fully secure online transition of information from you or to you. Upon receipt of personal information, the service will endeavour to store information securely and prevent unauthorised access.

Holding your personal information
By law, mental health records must be retained 20years after the last contact with the patient. When you no longer have contact with our service, it will securely archive your information.

Your data protection rights
Under the Data Protection Act 1998, and General Data Protection Regulation (GDPR) (2018) your rights relating to your personal information are as follows:

Your right of access - You have the right to ask us for copies of your personal information.
​
Your right to rectification - You have the right to ask the service to rectify factual information you think is inaccurate. You also have the right to ask the service to complete information you think is incomplete.
​
Your right to erasure - You have the right to ask the service to erase your personal information when there is no active need to process the information except where medical records need to be maintained as per our statutory duties. Though the service can not alter medical records, addendums may be included citing your disagreement to the records.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
​
Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances and subject to discussions as above.
​
Your right to data portability - You have the right to ask that we transfer the personal information you gave the service to another organisation, or to you, in certain circumstances.

Should you wish to exercise these rights you are entitled to make a ‘Data Subject Access Request’. please contact us at sjeewa@protonmail.com. You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
